Not all cases of lumbar scoliosis cause immediate symptoms. Mild forms of lumbar scoliosis often cause only cosmetic issues—such as visible asymmetry of the waist or pelvis. In more severe cases, however, typical symptoms develop that significantly affect daily life.
Local symptoms:
Back pain is the most common symptom of lumbar scoliosis. Lumbar scoliosis often causes severe back pain, which worsens with prolonged standing and walking.
Tension in the back muscles and a palpable lumbar bulge on the convex side are among the common findings.
The core muscles tire more quickly, and the pelvis tilts to one side.
Radiating and neurological symptoms:
Pseudoradicular pain in the buttocks and thighs occurs when muscle tension irritates the nerves in the back.
Radicular symptoms can result from spinal stenosis—for example, at the L4/L5 or L3/L4 levels. These symptoms manifest as pain, numbness, or weakness in the leg. Spinal canal stenosis occurs when the spinal canal is narrowed due to misalignment and wear and tear, putting pressure on the nerves or spinal cord.
Spinal claudication: The distance a person can walk is limited; relief occurs when sitting or bending forward.
Long-term consequences:
Premature wear and tear of intervertebral discs, vertebral bodies, and facet joints due to changes in the distribution of stress.
Progressive postural abnormalities, including pelvic tilt, leg length discrepancy, and excessive strain on the hip and knee joints. In severe cases, a rib hump may also become visible if the scoliosis extends into the thoracolumbar junction.
Lumbar scoliosis can cause limitations in movement—from climbing stairs to sitting for long periods at the office.

Diagnosis of Lumbar Scoliosis
The diagnosis of lumbar scoliosis consists of three components: medical history, physical examination, and imaging. Scoliosis is classified based on the level of the primary curve—the exact location and degree of the curve determine the subsequent treatment plan.
Clinical examination:
The Adams Bending Test helps detect scoliosis: When the patient bends forward, a lumbar bulge or trunk asymmetry becomes visible.
Assessment of the shoulders, waist triangles, pelvic alignment, and leg lengths to determine the overall misalignment.

Imaging – X-rays:
X-rays show the curvature and severity of scoliosis. The standard procedure is a full-spine X-ray taken while standing in two planes (anterior-posterior and lateral).
The Cobb angle is used to measure scoliosis: It determines the degree of curvature based on the Cobb angle in the lumbar region (e.g., L1–L5) and documents the deviation from the vertical.

An MRI is used to visualize nerve structures—in particular, to evaluate intervertebral discs, the spinal canal, and nerve roots when stenosis or a herniated disc is suspected.

Assessment of bone stability and osteoporosis risk, supplemented by a bone density measurement (DEXA) if necessary.

The goal of most treatment approaches is not to completely straighten the spine. Rather, the focus is on reducing pain, improving function, and preventing further progression. Conservative and surgical options are described separately below.
Conservative Treatment of Lumbar Scoliosis
Conservative treatment is based on physical therapy and pain medication. It is particularly suitable for mild to moderate scoliosis and stable cases.
Physical Therapy:
Scoliosis-specific exercises (e.g., based on the Schroth principles) involving three-dimensional postural correction and targeted breathing techniques.
Strengthening the deep back and abdominal muscles to stabilize the lumbar spine.
Stretching of shortened muscle groups on the concave side and mobilization of the jammed vertebral segments.
Exercise and Daily Life:
Regular, low-impact exercise: swimming, Nordic walking, cycling, or modified yoga and Pilates.
Ergonomic adjustments in daily life and at work—height-adjustable desk, back-friendly lifting and carrying, and regular breaks to stand up.
Pain Management:

Stabilization of unstable vertebral segments through spondylodesis. Corrective spondylodesis stabilizes the spine following correction and fuses the treated segments.
Typical surgical procedures:
Microsurgical decompression alone for localized narrowings without significant instability.
Decompression plus fusion surgery (corrective spondylodesis) using screw-rod systems and interbody cages (e.g., TLIF, PLIF in the L3–L5 region).
Long-segment fusion for severe, multi-segmental deformities, with the fusion boundaries defined at stable segments of the spine.
Risks and Complications:
Common risks include infections, postoperative bleeding, implant loosening, pseudarthrosis, and neurological complications.
There is an increased risk in older adults, those with osteoporosis, and those with relevant comorbidities.
Depending on the complexity of the procedure, studies report complication rates of 15–49% and reoperation rates of 14–15%—figures that underscore the importance of careful assessment of the indication and, in many cases, seeking a second opinion before spinal surgery.
Expected results:
Surgery often significantly reduces scoliosis, but it does not necessarily straighten the spine completely.
Typical improvements include reduced pain, increased walking distance, improved posture, and better ability to perform daily activities. In studies, the Oswestry Disability Index improved by an average of about 23 points following surgical treatment.
Example: A 65-year-old female patient with degenerative lumbar scoliosis (Cobb angle of 42 degrees), spinal stenosis, and increasing leg pain: After exhausting all conservative treatment options, a decompression and fusion surgery is planned to relieve pressure on the nerves and correct the instability.
Follow-up Care, Rehabilitation, and Daily Life with Lumbar Scoliosis
Postoperative Period:
Early mobilization: Getting out of bed on the first postoperative day, walking exercises on the ward.
Participants learn back-friendly movements—such as standing up, turning, and sitting correctly—under guidance.
Typical length of hospital stay: 7 to 12 days. This is followed by a rehabilitation phase, such as a 3-week inpatient program or intensive outpatient rehabilitation.
Long-term rehabilitation:
